When did it happen?

The day it happened, and the day you found out if that was later. One of the two is enough.

The day of the act or omission the claim is about: the missed payment, the accident, the work done badly.

The day you first knew there was a loss, that someone caused it, who, and that going to court was a sensible way to fix it. Leave blank if it is the same day it happened.
